Introduction

Most women with PCOS are told they have a hormone problem. And they do - but for many, the hormonal disruption isn't where the story starts. It starts earlier, with chronically elevated insulin. Understanding that distinction matters, because it changes what you treat: if you chase the hormonal symptoms without addressing the high insulin underneath, the symptoms tend to persist no matter what you do.

High insulin doesn't just affect your blood sugar. It directly disrupts ovarian function, hormone production, metabolism, and appetite - which is why symptoms like weight gain, fatigue, acne, and irregular cycles so often persist even when you feel like you're doing everything right. What high insulin actually is

High insulin - known clinically as hyperinsulinaemia - is when your body produces more insulin than it should need to manage your blood sugar. It's most commonly driven by insulin resistance: when your cells stop responding effectively to insulin, your body compensates by producing more of it, and your insulin levels stay elevated for longer than they should.

This is the part that's so often missed: you can have high insulin while your blood sugar still looks completely normal. In fact, that's the typical early picture - your body is successfully keeping your blood sugar in range precisely because it's pumping out extra insulin to do it. The blood sugar reads fine; the insulin behind it is anything but. How high insulin disrupts your hormones

High insulin isn't a passive background issue. It actively drives the hormonal imbalance that defines PCOS, through several distinct mechanisms.

1. It stimulates excess androgen production

This is the central one. Elevated insulin signals the ovaries to produce more androgens - male hormones such as testosterone. That androgen excess is what drives the most recognisable PCOS symptoms: acne, excess facial or body hair, scalp hair thinning, and disrupted ovulation.

The implication is significant and frequently overlooked: in PCOS, high testosterone is usually a downstream effect of insulin, not the primary cause. So treatments aimed only at lowering androgens are addressing a symptom of the problem rather than the problem itself. 2. It disrupts ovulation

High insulin interferes with the normal signalling that drives ovulation. When ovulation doesn't happen regularly, progesterone stays low and cycles become irregular or unpredictable - the menstrual disruption so many women with PCOS experience. 3. It promotes fat storage and blocks fat burning

Insulin is a fat-storage hormone. When it's chronically elevated, fat storage increases, fat burning is suppressed, and weight loss becomes significantly harder. This is the physiology behind the experience of doing everything right and still not losing weight - it's not effort that's lacking, it's that the insulin signal is holding the body in storage mode.

4. It drives cravings and energy crashes

High insulin produces unstable blood sugar, which shows up as strong sugar and carbohydrate cravings, energy dips after eating, and increased hunger. These effects don't stay separate - they feed each other. Insulin rises, androgens rise, ovulation is disrupted, metabolism slows, and insulin resistance worsens, which pushes insulin higher still. This self-reinforcing loop is why PCOS symptoms so often feel like they're slowly compounding rather than holding steady, and why intervening at the level of insulin - the start of the loop - is more effective than chasing each symptom individually.

Why high insulin is so often missed

Many women with PCOS are told their blood sugar is "normal" and reasonably conclude nothing is metabolically wrong. But there are three reasons that reassurance is misleading:

  • Standard tests rarely measure insulin directly. Fasting glucose and HbA1c measure sugar, not insulin - so the elevated insulin is simply never seen.
  • Insulin resistance can exist for years before blood sugar shifts at all, because the extra insulin keeps glucose in range the whole time.
  • Symptoms appear well before any abnormal blood sugar result - meaning you can feel the effects of high insulin long before a standard test would flag anything.

By the time blood sugar itself becomes abnormal, the underlying metabolic dysfunction is usually well established. This is why testing fasting insulin - not just glucose - matters so much, and why so many women spend years being told they're fine while the actual driver of their symptoms goes unmeasured.

What actually lowers insulin in PCOS

The goal isn't extreme dieting - it's metabolic stabilisation: bringing insulin down by reducing the spikes that drive it up and supporting your cells' sensitivity to it. The foundations that help most women:

Prioritise protein and reduce refined carbohydrates. Together these flatten the insulin response across the day, which is what gives your system the chance to recover its sensitivity.

Eat in a structured pattern rather than grazing. Spacing meals allows insulin to fall between them - and it's during those lower-insulin windows that your metabolism can begin to reset.

Why the right approach is specific to you

There is no single best way to eat for lowering insulin, because no two women produce the same insulin response to the same foods.

This is well established in nutrition science. Research tracking how thousands of people respond to identical meals has shown that the same food can trigger a sharp insulin spike in one woman and a calm, moderate response in another - and the difference comes down to individual factors: genetics, gut microbiome, hormonal status, muscle mass, and existing metabolic state.

So two women with PCOS can follow exactly the same "low-insulin" diet and get different results. A food that's genuinely helpful for one woman's insulin can quietly be spiking another's. This is why the most effective way to lower insulin isn't a generic plan - it's identifying the specific foods that produce a calm insulin response in your body and building your nutrition around those. Clinical Insight

Hyperinsulinaemia is increasingly recognised as a primary driver of PCOS pathophysiology rather than a secondary feature, and in a substantial proportion of women it precedes and precipitates the hormonal disturbance for which the condition is named.
The mechanism is well characterised: insulin resistance drives compensatory hyperinsulinaemia, and elevated insulin acts directly on ovarian theca cells to stimulate androgen synthesis while simultaneously suppressing hepatic sex hormone-binding globulin, raising free testosterone further. The resulting hyperandrogenism produces the clinical hallmarks - hirsutism, acne, androgenic alopecia, and ovulatory dysfunction - meaning that elevated androgens in PCOS are frequently downstream of insulin rather than independent.
Insulin additionally promotes lipogenesis and inhibits lipolysis, biasing the body toward fat storage, and contributes to the appetite dysregulation and glycaemic instability that drive cravings and post-prandial fatigue. Critically, this entire cascade routinely escapes detection, because standard screening measures glycaemia (fasting glucose, HbA1c) rather than insulin, and these remain within normal range throughout the compensatory phase - which can persist for years - while fasting insulin and HOMA-IR, the markers that would reveal the disturbance, are rarely ordered.
The clinical consequence is that many women are reassured their blood sugar is normal while the insulin driving their symptoms goes entirely unmeasured. In practice, interventions that lower the chronic insulin load are frequently associated with improvement across the symptom profile concurrently - a pattern consistent with the resolution of a shared upstream driver rather than the management of discrete symptoms - though individual response varies.
Because glycaemic and insulinaemic responses to specific foods differ substantially between women, nutritional intervention calibrated to the individual's biochemistry consistently outperforms generic dietary templates in achieving sustained reduction in insulin and the hormonal improvements that follow.
